PATIENT INFORMATION

Your privacy on these pages.

You can explore care topics without creating an account or entering health details. This notice explains this informational website—not a future patient portal or clinical record system.

An illustration of preparation notes and a checklist, not an actual patient record.
Original illustration · not actual staff, patients or records.
Keep health information out of this siteThe learning tests, preparation checklist, insurance selector and automated guide do not ask for personal health information. Do not send medical records, symptoms, member IDs, dates of birth or payment details to the website mailbox.

What the page tools do

Knowledge-test choices, preparation checkmarks, guide selections and broad insurance-selector choices operate in your current page’s memory. The tools do not send answers to a server or intentionally save them in cookies, local storage or a patient record.

Choices clear when you leave or reload the page. The tools do not perform a live insurance lookup, submit a message, schedule care or train a medical model on your answers.

This site does not include an analytics tracker, advertising pixel, account login, health-data upload or patient intake form.

Ordinary website requests still happen

Your browser requests pages, scripts, fonts and images to display the site. Those routine requests can expose technical information such as an IP address, requested URL, request time, browser information and errors to the hosting infrastructure.

Hosting, security and browser logs or caches may retain routine technical information. “Answers are not saved by our tools” does not mean that visiting a website creates no technical records anywhere. This notice does not promise a universal log-retention period or an absence of ordinary server logging.

A fragment used to navigate within a page is different from a submitted form. Do not put sensitive information in URLs or share links containing personal details.

Not a patient privacy notice

This is not a HIPAA Notice of Privacy Practices for a treating provider. It does not establish a clinical entity, a business-associate agreement or a claim that every future system is HIPAA compliant.

Before any clinical information is collected for future care, the responsible provider should identify itself and provide the applicable privacy notice, approved secure channels and information about records and patient rights.

For nonclinical questions about this website notice or accessibility, contact info@ozivita.com without including health information.
