Insurance preparation — nothing is submitted

Which insurance company is listed on your card?

These names help identify your plan. They are not a list of accepted insurance companies.

Other ways to continue

Selecting an insurance company does not confirm coverage. Your specific plan and visit details need to be reviewed.

No insurer is contacted. No answers are transmitted or stored. Close or restart to clear choices. Website privacy

Identification is not coverage confirmationA general insurance policy does not establish a contract with your exact plan, an in-network provider or payment for a service. No insurer is contacted by this page.

Five details to confirm together

  • Exact plan: the product and network—not just the insurer’s brand.
  • Treating provider: the individual clinician and billing practice your plan recognizes.
  • Service: what is covered, including telehealth, referrals and prior authorization.
  • Location: where you will be physically present and whether both care and coverage are permitted there.
  • Your benefits: deductible, copay, coinsurance, limits and exclusions for the date of care.

A useful call to your insurer

Use the member-services number on your own insurance card or your plan’s official member site. Keep personal identifiers out of this website and ordinary email.

Ask: “Is this exact provider in network for my plan, for this service by telehealth in my location? Do I need a referral or prior authorization? What could I owe?”

Keep the insurer’s confirmation and reference number for yourself. An estimate or authorization is not an unconditional payment guarantee.

Public coverage needs its own confirmation

Medicare, Medicaid and TRICARE each need exact program, plan and provider review. A program name does not establish enrollment, benefits or participation.

Benefits, clinician eligibility and state service availability remain separate checks. This page cannot establish any of them.

Know the costs before care

Fees and self-pay arrangements are not quoted here. Ask what a visit includes and whether labs, medicines, follow-up, referrals or care elsewhere would be billed separately.

Review applicable financial terms before agreeing to care. Do not send payment-card or insurance details to info@ozivita.com; that address is for nonclinical website questions.
